How to make an appointment

Established patients may call the main office number, (706) 722-4300, and speak to the receptionist or leave a message indicating the time and date that you would like to have your appointment.  You may also manage your appointments online through MyChart. For those who are new to our practice, your obstetrician must refer you for consultation. Please use this referral form.
